The Damien Center is a leader in HIV/AIDS prevention and care in Central Indiana. Dining Out for Life is the Damien Center's annual fundraiser. They needed a way to reach area restaurants to inform them about event, plus a registration form for those who wished to participate. We accomplished this by designing a direct mail brochure which promoted the event and described how restaurants could participate. The brochure featured a perforated flap with the registration form on the reverse side that restaurants filled out and mailed back. Participating restaurants on the night of the event donate 25% of their profits to the Damien Center. Proceeds from the night help pay for local services in the Fight Against AIDS.
